On January 29, 2025, at approximately 2048 EST, a UH-60 Black Hawk helicopter operated by the United States Army, and American Eagle Flight 5342, a Bombardier CRJ700 regional jet, collided during the jet's final approach to Ronald Reagan Washington National Airport (DCA). Both aircraft crashed into the Potomac River, resulting in the deaths of all 64 passengers and crew members aboard the jet, as well as three soldiers on the helicopter. Nearly one year later, the National Transportation Safety Board (NTSB) released its final report following a board meeting on January 27, 2026.
The NTSB determined that the probable cause of the accident stemmed from three primary failures by the Federal Aviation Administration (FAA): the placement of a helicopter route in close proximity to a runway approach path at DCA; the FAA's failure to regularly review helicopter routes, analyze safety data, and act on prior recommendations to reduce midair collision risk; and the air traffic system's overreliance on visual separation, without adequately accounting for its limitations.
The Board also identified more than 70 additional causal and contributing factors. These included the helicopter crew's failure to maintain visual separation from the jet, high controller workload that led to a loss of situational awareness, the Army's failure to ensure pilots understood barometric altimeter error tolerances which contributed to the helicopter exceeding its maximum authorized altitude, and radio reception issues minutes before the collision that prevented the helicopter crew from receiving transmissions that the jet would be circling before landing on Runway 33.
Questions arose as to why the investigation required nearly a year to complete. The International Civil Aviation Organization's Manual of Aircraft Accident and Incident Investigation (Doc 9756, Part IV — Reporting, Section 1.5.1) recommends that "The State conducting the investigation shall release the Final Report in the shortest possible time and, if possible, within twelve months of the date of the occurrence." While that recommendation was met, NTSB Chair Jennifer Homendy criticized the FAA's response, noting that the agency was slow in providing requested data and may have attempted to limit the scope of certain investigative areas. Homendy stated, "You're too late. We've already figured it out ourselves," in reference to delays in receiving information. The NTSB also praised the overall cooperation of the FAA and the U.S. Army's responsiveness during the investigation.
Days prior to the release of the final report, Transportation Secretary Sean Duffy announced that the FAA would formalize permanent restrictions for helicopters and powered-lift aircraft operating near DCA. These restrictions, supported by NTSB preliminary recommendations, were implemented through an Interim Final Rule (IFR) under 14 CFR Part 93 (Docket No. FAA-2026-0199; Amdt. No 93-105), published January 23, 2026.
"These revisions are necessary to mitigate the potential for midair collisions resulting from insufficient separation between fixed-wing aircraft operating to or from DCA and helicopters or powered-lift operating in the vertical-lift flight mode near DCA. The revisions will also fully implement a National Transportation Safety Board safety recommendation. The flight restrictions will prohibit helicopters and powered-lift operating in the vertical-lift flight mode from operating in a designated area near DCA unless these aircraft are conducting an essential operation."
The FAA has continued to take additional actions to enhance safety, including revising agreements with the military to require ADS-B Out broadcasting, eliminating the use of visual separation within five nautical miles of DCA, modifying helicopter zones and routes, increasing staffing and operational support at the airport, and establishing procedures to reduce mixed helicopter and fixed-wing traffic near DCA.
While these steps represent significant regulatory and operational changes, aviation safety experts caution that the effectiveness of these measures will be determined over time. The NTSB report and subsequent FAA actions provide both accountability and hope that lessons learned will prevent future tragedies.
